Calls, Advocacy, and Resources for Elders

About Us

The goal of CARE Connections is to serve low-income older adults in the Durham community and expand access to resources related to health, nutrition, housing, and transportation. We accomplish this goal through weekly well-check calls, resource connection, food delivery, education sessions, and health fairs. The weekly well-check calls are the foundation of the program and volunteers are paired with seniors to provide calls with the goal of establishing an authentic and trusting relationship. These calls allow students to better understand community members outside of the clinical setting and identify areas to address social drivers of health. Please fill out the above form or email dukemedicinebecfellows@dm.duke.edu if interested in getting involved!
